New application form for a temporary residence permit

The new form template was introduced by the Regulation of the Minister of Internal Affairs and Administration of 25 November 2025 on applications for temporary residence permits for foreigners (Journal of Laws, item 1647).

The need to issue it resulted from changes introduced by the Act of 24 April 2025 amending the Act on Foreigners and certain other acts (Journal of Laws, item 619) and the Act of 20 March 2025 on the conditions for the admissibility of entrusting work to foreigners on the territory of the Republic of Poland (Journal of Laws, item 621).

The Regulation specifies:

• a template of the application form for issuing a temporary residence permit to a foreigner, referred to in Article 106(1) of the Act on Foreigners;
• a template for the attachments to the application referred to in Article 106(1a) and (1c) to (1e) of the Act;
• a template for the applications referred to in Article 139a(1) and Article 139o(1) of the Act;
• the number of photographs to be attached to the application;
• detailed technical requirements for photographs attached to the application;
• a template of the stamp confirming the submission of the application;
• the method of taking fingerprints for the purpose of issuing a residence card;
• the method of recording the data entered in the residence card and transferring it for the personalisation of the residence card.
