Starting July 2025, a new version of the application form for the EU long-term resident permit is in force.
This update aligns Polish regulations with EU law and simplifies procedures for foreigners residing in Poland.
Where to download the new form?
The new application form is available:
- on the website of the Office for Foreigners: https://www.gov.pl/web/udsc/nowy-wzor-wniosku-o-udzielenie-zezwolenia-na-pobyt-rezydenta-dlugoterminowego-ue
- under Documents to Download – Zezwolenie na pobyt rezydenta długoterminowego UE
- in paper form — at the information point of the provincial office (voivodeship office) responsible for the place of residence of the foreigner
Note: Fill in the form clearly — preferably on a computer — then print and sign it.
Who can apply? The permit may be granted to a foreigner who:
- has been legally and continuously residing in Poland for at least 5 years,
- has a stable and regular source of income,
- holds health insurance,
- has Polish language proficiency at least at B1 level (confirmed by a certificate).
Required documents
The application must be accompanied by:
- A completed application form (new version!)
- 4 recent passport-style photos
- A copy of a valid travel document (e.g. passport)
- Documents confirming legal and continuous stay in Poland for at least 5 years
- Proof of stable income for the past 3 years (employment contracts, PITs, tax office certificates)
- Proof of health insurance (e.g. from ZUS)
- Certificate confirming knowledge of Polish (minimum level B1)
- Proof of payment of the application fee (PLN 640)
- Proof of accommodation
- Proof of health insurance contributions for the last 3 years (ZUS).
Where to submit the application?
The application must be submitted in person at the provincial (voivodeship) office competent for the foreigner’s place of residence.
Why is it worth applying?
The EU long-term resident status provides greater residence stability, easier access to the labour market, and facilitates travel within the European Union.
